Communication Procedures

The most popular question for us when speaking to a new dentist office that wants to work with us is "What do you need us to send you for ____ type of case?" The next few pages will explain what we need, give some examples of why we need these things, and what could happen if we don't get everything we need.

Diagnostic Wax-Ups

For a Diagnostic Wax-Up you would need to send us a Full Mouth Impression with HIP (with no pulls) keep in mind the better the impression the better the provisional, Smile Catalog or patients gender so we can determine shape of teeth, Occlusal Classification (I II or III), Shimbashi, and Articular Eminence Slope.

Small Unit Cases

If you are sending us a Small Unit Case (three or less) we need you to send a full arch impression (with no pulls). If you only send a partial impression we cannot guarantee fit or accuracy of occlusion. We need a good, clean Opposing model. And we need the material to use and Shade, if custom shades use Shade- Connection.

Multiple Unit Cases

And if you are sending us a Multiple Unit Case (four or more) we need the following: a Full impression with HIP (with no pulls), a good, clean Opposing Model, a Bite Record and Shimbashi, material preference, and Shade (if custom shades use Shade Connection).

Full Arch Cases

In the instance you are sending us a Full Arch Case, we need the following: a Full Impression with HIP, a good Opposing Model, a Bite Record & Shimbashi, Articular Eminence Slope, Model of Temps, material preference, Checklist for EDOT, and Shade with optional smile catalog.

Full Mouth Reconstruction Cases

Say you are sending us a Full Mouth Reconstruction Case, we would need you to send us a Full Mouth Impression with HIP (with no pulls), Models of provisions, Bite Record of upper to lower prepared teeth with shimbashi, Articular Eminence Slope, checklist for EDOT, Shade Connection with optional smile catalog.

Dentures

In the case you are sending us a Denture; we need a Full Mouth Impression with HIP (with no pulls), the Alameter and Papillameter Readings to make the bite rim. To set the teeth properly we need Occlusal Classification, Articular Eminence Slope, the "F" Point Placement, and Shade & Mold.

Partials

Should you send us a case to do a Partial we need a Full Mouth Impression with HIP (with no pulls), the Alameter and Papillameter Readings (when applicable), the Dentist needs to describe the type of partial framework (i.e. FRS, Metal, or Thermo Flex Acetal Resin), Shade and Mold.

Orthotic Appliances

When sending us a case to do an Orthotic Appliance we need the following a Full Impression with HIP, a Shimbashi with Bite, and we need the dentist to indicate the type of appliance (i.e. long term Acetal [comes in tooth shades].

In Conclusion

All cases it is imperative that we receive a prescription that is thoroughly filled out and signed by the Dentist. It is also imperative that we receive impressions with the HIP and without pulls (Schreinemakers trays from Accu-Liner produce excellent impressions). In addition, giving us a correct Shimbashi helps us to verify that we mounted the models correctly.

In the event you send us a script that is not completely filled out or signed, we will attempt to contact your office as soon as we can.

If it is written on the script to "use your best judgment" or any phrase that indicates to us to do the best we can, and the patient or dentist decides they are not satisfied, we are not responsible.
